If you find it difficult to manage your health care needs due to chronic or long-term health issues, a health coach could be the right choice for you!
A health coach is a nurse who can assist you in navigating through various health care components necessary to facilitate your overall care. This includes enhancing your patient experience, ensuring your care needs are met, helping you manage your condition and contributing to a better quality of health outcome.
Through UMR’s Care Management Program, eligible Compass Rose Health Plan members receive expert support and resources that assist in improving and maintaining their health to bring chronic medical conditions under control.
UMR’s team of registered nurse coaches are available to eligible employees and dependents enrolled in the Compass Rose Health Plan. There is no cost to participate, and your personal health information is confidential.
Many conditions may require you to seek medical treatment from multiple providers over an extended period. This can create a nightmare scenario if you are trying to identify what steps you should take to get the treatment necessary for your specific care.
The program is open to members with one or more of the following conditions:
- Heart failure
- Heart disease
- Chronic obstructive pulmonary disease (COPD)
- Depression (when occurring with one of the other listed conditions)
It is important to remember that when you are able to manage your symptoms, you can:
- Improve your general health
- Reduce or eliminate the need for medications
- Lower your long-term health risks
- Avoid flare-ups that lead to ER visits, hospital admissions or readmissions
- Lower your out-of-pocket costs
UMR’s nurse coaches can assist you in making informed decisions to manage your symptoms as well as your overall health. In addition, they have helped participants gain control of their asthma, adjust their diet to lower their blood pressure, and lose weight to better manage their diabetes and reduce the need for medications. After receiving coaching sessions, you may receive periodic phone calls and informational materials to help you develop new, healthy habits.
UMR also provides useful, interactive tools online to help you reach your health goals.

- Track your progress using your own personal health record
- Participate in action plans that provide you with useful information and online activities to guide your journey to healthier living
Anyone who has a chronic condition can benefit from care management. If you are at a higher risk, UMR will contact you to participate in coaching sessions.
To find out if you are eligible and sign up for coaching, login to your Member Portal at https://member.compassrosebenefits.com. Choose ‘Claims & EOBs’ from the menu bar. Then, select the button corresponding with your current plan information, which will redirect you to your personal profile within UMR. Once there, go to the Health center and select ‘Get a health coach’ from the ‘I need to’ menu. You can also speak directly to someone at UMR by calling (888) 438-9135.
