Beginning January 15, 2022 the Compass Rose Health Plan will provide assistance on reimbursement of over-the-counter (OTC) COVID-19 tests that have been authorized or approved by the U.S. Food and Drug Administration (FDA).
Members and their covered dependents may receive 8 tests per 30-day period (or per calendar month). Each test is counted separately if multiple tests are sold in one package.
If you are a Compass Rose Health Plan member (without Medicare Advantage) you can receive reimbursement for an OTC COVID-19 test two ways:
- At a participating network pharmacy, such as CVS and Walgreens, you may receive your OTC at-home COVID-19 test(s) with no out-of-pocket costs to you. You can locate a network pharmacy through Express Scripts. You must present your health plan member ID card to receive this benefit.
- If you purchase an at-home COVID-19 test from any other in-store or online retailer, you may submit a claim for reimbursement. The maximum reimbursement is $12 per test. Many COVID-19 tests are sold as a 2-pack kit ($12 for each test) and would be reimbursed at a maximum of $24 per kit.
- To submit for reimbursement, complete the Express Scripts Claim Form. You must have a receipt with UPC, product/name, quantity, etc. NOTE: There are fields on the claim form that are not applicable in this instance (i.e. DAW, Rx number, Doctor Name). You may submit these claims electronically or via mail or fax as instructed on the form.
Please allow up to 30 days for the at-home COVID-19 test to be reimbursed.

Anyone can get OTC at-home COVID-19 tests from the Federal government: The Federal government has launched a national website where each household can receive a one-time shipment of four free OTC at-home Covid-19 tests shipped directly from covidtests.gov. Visit the website for more information.
